For massage sessions, please arrive 5 minutes early for a quick check-in so we can begin on time, and you can receive your full session. Your massage starts at your scheduled time.

POLICIES

LIABILITY:
CLIENT (hereinafter called "CUSTOMER") understands that Body, Mind, and Soul, LLC requires Professional Liability Insurance Coverage for each of its massage therapists and will provide detailed policy information & verification of said coverage to CUSTOMER upon request.

INDEMNITY:
CUSTOMER agrees to indemnify and hold Body, Mind, and Soul, LLC, harmless from any claim, loss, cost, expense, demand, or damage arising directly or indirectly out of the use of its on-site services and activities in connection therewith.

PAYMENT:
Payment is due in full on the date of service.

CANCELLATIONS:
Life ALWAYS happens; however, should you cancel within your 24-hour window, the first appt 1/2 of the session fee will be required (depending on the situation, full fee may be charged). If you no-show, your full session fee is required.

MISCELLANEOUS:
The Terms of Service and the relationship between CUSTOMER and Body, Mind, and Soul, LLC shall be governed by the laws of the State of Ohio without regard to its conflict of law provisions. Customer and Body, Mind, and Soul, LLC agree to submit to the personal and exclusive jurisdiction of the courts located within the state of Ohio.
